We supported the West Yorkshire ICB Health and Growth Accelerator Programme, a programme for people with multiple long-term conditions, including mental health, by gathering feedback from those affected and their carers. Their feedback offers insight into how these people can better self-manage their conditions, avoid hospital admissions, have more valuable, effective, and person-centred appointments, improve communication, and, in turn, improve their ability to work.

Take a look at the Multiple long-term health conditions report to understand what people currently find frustrating and how services can help.

Highlights

People with multiple long-term conditions told us they would like longer appointments with their GP or healthcare professional to be able to discuss all their health concerns at once.

Most people we spoke to have issues with booking GP appointments. Therefore, often wait until health problems are at a critical level and go to A&E.

Many people with multiple long-term conditions struggle to attend appointments due to transport costs, the time needed for mutliple appointments, clinics and services across the city.
